- This is a fully vaccinated party following CDC and NYC guidelines. Proof of full vaccination status is REQUIRED for entry.. If providing a vaccination card, it must be the ORIGINAL document. No exceptions. Acceptable proof of “full vaccination” are below: ⁃ A CDC-issued vaccination card including the name of the person vaccinated, the type of vaccination provided and the date that the last dose was administered. ⁃ A printed photo of a CDC-issued vaccination card. ⁃ A digital photo of a CDC-issued vaccination card stored on a phone or electronic device. ⁃ A digital vaccination record from your medical provider or pharmacy website or app. The following link will allow you to register for Excelsior Pass and only takes a few minutes to do so. If you were vaccinated in New York State, you are already in the database and simply need to activate your pass. https://epass.ny.gov/home
